From Dan's work & Google Maps, I believe the address of the Perry house is currently 130 Flanders Memorial Road. (Drive seems to have morphed into Road since 1988.) The bend in Flanders Mem Rd is right where I expected it to be, with respect to 130. While the footprint of the front buildings at 130 is consistent with the Perry place, there is a lot more going on there now - the woods have been cleared, there are several more outbuildings, and it looks like a business is or was being run there. The distance from 130 to the general store is 0.6 miles, consistent with my recollections - I definitely walked it when I was 11 or 12.

The plot plan from Aug 1988 showed the Hesslein place on Woods Road. That street name that does not currently exist. The track used to slope up and end at a field, past the Hesslein place. The 1988 plan shows the Hesslein place as Timothy & Linda Kennord; the current address is 134 Flanders Mem Rd. It's hard to tell from the satellite view, but it doesn't look like a log cabin any more.
